In my opinion Orlan is crazy, but that performance that she did "the French kiss" I think that is the most quiet art demonstration, I actually see it as a funny and as an original way to call people attention, and the results made me laugh a lot. When I started to watch the video, the other things that she did, and all that crazy ideas about controlling her own pain, I got static, and I can believe it yet. Maybe she got crazy when she lost her baby, for a woman it is a very hard thing.
Her society message? Well, to me it doesn't mean anything, besides that she is very brave for doing all those body changes and using her body as an art tool, I think that the thing that maybe can create an opinion in society, is her changes more than her art pieces, because maybe her art is interesting and even beautiful, but she is her own "art icon".
About the narcissism and exhibitionism in art, It is going to depend on your viewpoints, to agree or disagree with her statements, you have to have a very mature point of view, and not insult or offend her passion.
I agree with you, Orlan is crazy, I mean, she is a extremely crazy, but this world need people like her because life is funnier and interesting. However, I never will try to do things like she did.
